Phase II Cardiac Rehabilitation
Phase II refers to an outpatient, medically supervised program that is typically initiated 1 to 3 weeks after hospital discharge and provides appropriate electrocardiography monitoring. It is a comprehensive program that includes the following components:

A medical evaluation;
A program to modify cardiac risk factors such as nutritional counseling
Prescribed exercise, Education, Counseling
2 to 3 sessions per week for 12 to 18 weeks not to exceed 72 sessions within a 36 week period

Phase II Criteria

Have a documented diagnosis of acute MI within the preceding 12 months
Have had coronary bypass surgery
Have stable angina pectoris
Have had heart valve repair/replacement
Have had percutaneous transluminal coronary angioplasty (PTCA) or coronary stenting
Have had a heart or heart/lung transplant

Phase III Criteria (Phase III Criteria is very similar to Phase II with the following exceptions :)

Completion of Phase II Program is required
Patients are monitored for weight and blood pressure however; patients are not
Electrocardiography monitored.

Phase II Pulmonary Rehabilitation

A medical evaluation
A program to modify pulmonary risk factors
Such as nutritional counseling, smoking cessation
Prescribed exercise
Education
Counseling
2 sessions per week for 15 weeks; not to exceed 30 sessions within a 52 week period

Phase II Criteria

FEV 1.0 less than or equal to 65% predicted value
FVS less than or equal to 65% predicted value
Diffusing capacity for carbon monoxide adjusted for
Hemoglobin less than or equal to 65% predicted value
Resting hypoxemia (Spo2 less than or equal to 90%)
Exercise testing demonstrating hypoxemia (Spo2 less than or equal to 90%)
or ventilatory limit (VE/MVV than or equal to 0.8) or a rising Vd/Vt

Obstructive Diseases: Interstitial disease

COPD (including alpha-1 antitrypsin deficiency)
Persistent asthma
Bronchiectasis
Cystic fibrosis
Bronshiolitis obliterans

Interstitial fibrosis
Occupational or environmental lung disease
Sarcoidosis

Chest Wall diseases Neuromuscular diseases:

Kyphoscoliosis
Ankylosing spondylitis

Parkinson’s disease
Post-polio syndrome
Amyotrophic lateral sclerosis
Diaphragmatic dysfunction
Multiple sclerosis
Post-tuberculosis syndrome

Other Conditions Phase III Criteria

Lung Cancer
Pre and post thoracic and abdominal surgery
Pre and post lung volume reduction surgery
Ventilatory dependency
Pediatric patients with respiratory disease
Obesity related respiratory disease

(Phase III Criteria is very similar to Phase II with the following exceptions:)
Completion of Phase II Program is required
Patients are monitored for weight and blood pressure however; they are not electrocardiography monitored as they exercise
